Case management for youth development and after-school programs
Youth programs run on several funders at once, and each wants its own numbers. Collaborate keeps one record per student and gives each program line its own forms and reports.
One student, every program
- Enrollment captures the student, guardians, emergency contacts, household details and a recorded decision for each consent.
- Name and date of birth live on the student, so age stays current and a student in two programs is one person.
- Each program line is its own program: it sets who sees what and what each report counts.
“We’re using data to ensure all kids get the same level of service across the county.”

Case management that counts
- A guided intake keeps your interview prompts under each question. Report a safety concern and the description becomes required.
- Session notes are service records with a required start time, so case-management frequency is countable.
- Rubrics are scored per criterion. Exit notes ask follow-up questions based on the reason for leaving.
Surveys that line up
- Pre and post surveys share field names, so a pre-to-post comparison needs no mapping.
- Every statement is its own reportable answer, so you can see which one moved.
- Leadership sessions and projects are recorded as activities, and each registered participant gets their own pre and post survey link.
WIOA and other funders
- WIOA pre- and post-internship surveys are built as forms.
- Ask the reporting agent, the AI assistant that runs your funder reports, how many case notes each program logged this month. Collaborate computes the numbers, not the agent.
